Once we’ve completed our first prenatal and you’ve had time to think on your preferences, you’re welcome to fill out your birth preferences below.

Once submitted, I will email you a beautiful, customized birth plan via Canva that has been updated with your specific choices. You will still be able to edit this afterward, nothing is set in stone! If you do make changes, please just let me know so I can keep your most recent plan handy for your labor.

I recommend printing a physical copy for your hospital bag, and I will keep a digital version on my phone for easy reference during your birth.
